Job SummaryAs the CIN Care Navigation RN, you will coordinate and optimize care transitions for ACO and patients transitioning from acute care to post-acute care and ultimately, home. You will track patients transitioning into post-acute care facilities to ensure continued quality care. You will work with post-acute providers to monitor and improve patient outcomes.

Responsibilities- Coordinate and optimize care transitions for ACO and patients moving from acute care to post-acute care and home.
- Track patients transitioning into post-acute facilities to ensure continued quality care.
- Collaborate with post-acute providers to monitor and improve patient outcomes.
- Education: Associate or bachelor’s degree in nursing.
- Skills: Knowledge of case management and community resources. Ability to function independently in a fast-paced environment. Written and verbal communication skills. Ability to establish priorities and communicate effectively.

Experience:
At least 2 years of previous case management, care navigation, or post-acute care experience. - License: Active and unencumbered RN license in Ohio and Michigan - or willingness to obtain upon hire.
- Education: Master’s degree in nursing or other relevant healthcare field.
