Pay details: $70 per regular visit, $75 per evaluation/assessment and $100 per start of care.

At Home Quality Care was founded in 1983 in Morris, Illinois. We deliver comprehensive home health and non‑medical services throughout the region, with seven branches across Texas, Illinois, and Indiana.

This role requires visits in the Tomball, Texas area.

The physical therapist is responsible for providing physical therapy services and emphasizing health education. They travel to patients’ homes to administer services dictated by their physician, helping patients maintain independence and quality of life. The role offers flexibility.

• Provides physical therapy services to patients according to a written physician’s plan of care.
• Assesses and evaluates therapeutic/rehabilitative/functional status and participates in the development of the total plan of care; evaluates home environment.
• Assesses muscle strength, mobility, gait, ROM, and potential for rehab.
• Directs physical therapy treatment.
• Instructs patients and families/caregivers in the use and care of therapeutic appliances.
• Determines priority needs for physical therapy.
• Reports to physicians about patients’ reactions to treatment or changes in condition.
• Initiates the physical therapy program and instructs other personnel and/or family/caregiver members in certain phases of therapy.
• May train patients in the use of prosthetic devices.
• Identifies patient and family/caregiver needs for other home health services and refers as necessary.
• Prepares and submits clinical and progress summaries based on attainment of goals.
• Participates in discharge planning for patients.
• Provides physical therapy consultation to home families/caregivers when indicated.
• Provides in‑service education programs for nursing organization personnel as needed.
• Participates in peer consultation processes.
• Supervises physical therapy assistants according to organization policy and state regulations.
• When therapy is the only skilled service, instructs, supervises, and evaluates home health aide care every two weeks.

• Possesses a degree from a baccalaureate or Master’s program in physical therapy approved by an accredited organization.
• Licensed to practice as a physical therapist within the state, if applicable.
• Two years of appropriate experience as a physical therapist; community/home health experience is preferred.
• Demonstrates good verbal and written communication and organization skills.
• Possesses and maintains current CPR certification.
• Must be a licensed driver with a vehicle that is insured and in good working order.
