Job OverviewWe are seeking a highly organized and compassionate Full-time Intake Coordinator to join our dynamic team. The ideal candidate will play a crucial role in managing the intake process for new patients, ensuring a smooth transition into our care services. This position requires excellent communication skills, attention to detail, and a strong understanding of home health care processes.
Key Responsibilities- Manage the intake process for new patients, including initial assessments and documentation
- Serve as the primary point of contact for patients, families, healthcare providers, and insurance companies
- Conduct thorough interviews with patients and/or their families to gather essential information
- Verify insurance coverage and obtain necessary authorizations for services
- Coordinate with clinical staff to develop appropriate care plans based on patient needs
- Schedule initial home visits and coordinate care team assignments
- Maintain accurate and up-to-date patient records in electronic health record systems
- Ensure compliance with all relevant healthcare regulations and company policies
- Collaborate with interdisciplinary teams to facilitate seamless patient care
- Handle sensitive patient information with the utmost confidentiality and professionalism
- Assist in resolving any issues or concerns raised by patients, families, or healthcare providers
- Generate reports and analyze data to improve intake processes and patient outcomes
- Participate in quality improvement initiatives and contribute to the development of best practices
- Stay informed about industry trends and changes in home health care regulations
Required:
- Bachelor's degree in Healthcare Administration, Nursing, or related field
- Minimum of 2 years of experience as an Intake Coordinator in a healthcare setting
- Proficiency in electronic health record systems and Microsoft Office Suite
- Strong understanding of medical terminology and home health care processes
- Excellent verbal and written communication skills
- Exceptional organizational and time management abilities
- Ability to multitask and prioritize in a fast-paced environment
- Detail-oriented with a strong commitment to accuracy
- Knowledge of healthcare regulations, including HIPAA compliance
- Empathetic and patient-centered approach to care coordination
- Previous experience in home health care or related field
- Familiarity with insurance verification and authorization processes
- Certification in case management or related field
- Experience with quality improvement initiatives in healthcare
- Strong interpersonal skills
