Job Overview: In this key role, the Pharmacist is responsible for the provision of pharmaceutical care to all patient age groups, including geriatric, adult, adolescent, pediatric and neonatal patients.

• Graduate of an ACPE accredited college of Pharmacy, or foreign equivalent educational degree

• Completion of hospital pharmacy residency and/or advanced pharmacy degree preferred
• Current New Jersey State license as a Registered Pharmacist in good standing
• Familiarity with all phases of Hospital Pharmacy and a working knowledge of public health matters, as well as, Federal and State laws regulating the professional and hospital practice of pharmacy
• Understands the role of the Hospital Pharmacy Department within the hospital and its interrelationships with other departments and functions
• Promotes positive relations and actively contributes to a positive work environment

• This individual will prepare, dispense, and monitor both the prescribing and effectiveness of all medications, including chemotherapy and IV admixtures, while exercising considerable independent decision-making ability.
• Utilizes the medication screening process to ensure medication orders are complete, appropriate, within legal parameters and prioritized
• Evaluates each medication order for formulary status, appropriate dose, dosing interval, allergy, drug/drug interaction, and drug/disease state interaction
• Oversees and prepares parental admixtures, dispensing, and compounding of medications
• Contacts physician directly for orders requiring intervention and maintains appropriate entries in intervention log
• Notifies nurse of any changes/modifications of medications prescribed
• Displays self-initiative in anticipating and resolving daily work issues
• Supervises technical staff to ensure accuracy and compliance with policies and procedures
• Participates in ongoing education and training related to organization and department specific requirements
• Properly complies with all national patient safety goals/risk reduction processes and associated policies and procedures as applicable to medication management and the pharmacy setting
• Participates in medication error and adverse drug reaction reporting programs
• Assists with orientation and training of new employees, students, nursing and medical staff
• Promotes positive relations and actively contributes to a positive work environment
