Facility: Trinitas Regional Medical Center, Elizabeth, NJ 08755

Shift: Evening (3P-11P)

Pay Range: $20.96 - $29.60 per hour

The Community Health Worker will maintain a presence in the community and work with patients to promote healthy living. The CHW will assist patients in utilizing resources and accessing services to overcome barriers in order to meet their health goals.

• Meet patients in clinic, hospital, ER, and community locations and conduct needs assessment using the Social Determinants of Health Tool.
• Schedule weekly follow-up calls and home visits to patients.
• Help patients with social issues such as homelessness, substance abuse, hunger, transportation, and other barriers impacting their lives.
• Assist patients with organizing records and scheduling follow-up appointments with primary care physicians or specialists.
• Support patients with tasks related to medical care and working toward health goals by planning healthy meals and finding time to exercise.
• Help patients access resources including quality of care, childcare services, food, housing, and transportation.
• Maintain relationships with community-based organizations.
• Travel to meet patients at churches, homeless shelters, and private residences as needed.
• Duties may change at any time with or without notice.

• High School Diploma or equivalent.

• Full-Time, Evening 3P-11P.
