Responsibilities

Powerback is a new way of delivering recovery, rehabilitation, respiratory, and wellness services to patients across the nation. The Physical Therapist will primarily provide outpatient rehabilitation services in an in‑home setting for the older adult population. The role includes evaluating and treating patients in the most functional environment—typically the patient's home—developing individualized therapy programs, making referrals to other disciplines, and instructing patients and caregivers in essential skills to promote independence and productivity.

  • Supervise Physical Therapy Assistants, temporary licensees, students, and support personnel, in accordance with state licensure requirements and professional standards.
  • Ensure adherence to department and patient treatment schedules, timely clinical and billing documentation, and participation in work‑related meetings.
  • Attend and contribute to patient care, staffing conferences, and other related meetings.
  • Make referrals to other disciplines and agencies to address the comprehensive needs of the patient.
  • Consult with other rehabilitation team members to select the most appropriate therapeutic regimen consistent with each patient’s needs and capabilities.
  • Adapt program and/or plan of care according to the needs of the individual patient.
  • Promote a safe environment and enforce the elimination of fire and safety hazards.
  • Order supplies and equipment as necessary.
  • Clean, maintain, and report needed repairs of equipment.
  • Ensure that patients and families receive the highest quality of service in a caring and compassionate atmosphere which recognizes the individuals’ needs and rights.
  • Screen, examine, and evaluate patients—including history, systems review, and application of appropriate tests and measures—interpret findings, establish a diagnosis, identify impairments, determine predicted level of improvement, identify precautions/contraindications, and design and plan of care.
  • Design appropriate treatment goals and methods in collaboration with the patient and caregivers, implement the physical therapy treatment plan, and complete all related documentation and record‑keeping.
