Purpose: We are looking for an experienced Nurse Practitioner to provide primary and palliative care to homebound patients in the Ocean County, NJ region. You will apply medical techniques and principles under physician supervision and care for a diverse set of patients with chronic diseases.
What You'll Do:- Provide high‑quality care to primary and palliative patients in and around Ocean County, NJ.
- Assess patient health by performing physical examinations, updating medical histories, and creating care plans.
- Effectively communicate with office staff and clinical operations across internal platforms.
- Provide patient‑centered care to a panel of patients.
- Attend quarterly medical staff meetings.
- Develop impressions, diagnoses, and treatment plans.
- Foster trust and consult with patients, families, and caregivers during visits.
- Order diagnostic testing as medically necessary.
- Prescribe medications and medically necessary interventions.
- Board‑certified Nurse Practitioner or Physician Assistant with a valid New Jersey license.
- Strong clinical skills with the ability to treat, assess, and diagnose confidently.
- Positive demeanor and strong interpersonal skills.
- Ability to problem‑solve and operate independently.
- 1–2 years of primary‑care experience with geriatric patients (hospice experience highly preferred but not required).
