Advanced Practice Provider role at Astera Cancer Care

Overview:
The Advanced Practice Provider is responsible for the evaluation and treatment of patients with hematology and oncology conditions. The role works within a multidisciplinary patient care team under the direction of a physician and collaborates with clinical and support staff to ensure safe, effective, quality, person-centered care. This position will be based in the New Jersey locations (Toms River/Brick).
- Provides all care in accordance with state and federal law as well as PCI policies, standards of practice, and prescribed workflows. In collaboration with a physician.
- Conducts patient interviews and physical evaluations to evaluate and treat new and existing patients.
- Orders and/or performs laboratory tests.
- Orders and/or performs diagnostic and/or therapeutic procedures and testing.
- Collects and interprets diagnostic data.
- Diagnoses medical problems, develops care plans, and prescribes treatments.
- Arranges for consultations with or referrals to other medical professionals or community services.
- Educates patients on health habits, self-examinations, behavior management, follow-up assessments, at-home care, medications, and other health-related issues.
- Utilizes available software to document patient histories, assessments, interventions, and interactions.
- Provides psycho-social and emotional support to patients and care partners.
- Recognizes and uses the roles of the interdisciplinary team, patient and family care partners as resources in planning patient care.
- Provides clinical information to the Revenue Cycle Management Department as needed for claims and authorizations.
- Participates in prescribed research training to develop understanding of clinical trials and applies knowledge to trial-related tasks.
- Communicates effectively with patients, care partners, healthcare team members, and third-party agents or organizations.
- Serves as a resource to colleagues and as a preceptor to new employees as directed by organizational leadership.
- Uses resources responsibly with a focus on cost containment.
- Committed to professional development and ongoing learning.
- Participates in professional activities to stay current with trends and developments in the field.
- Maintains confidentiality of information and demonstrates HIPAA proficiency.
- Attends required trainings and participates in organizational committees as directed.
- Performs related work as directed by management.
- Graduate of an accredited Nurse Practitioner or Physician’s Assistant program
- Active certification as a Nurse Practitioner or Physician’s Assistant
- Active NJ licensure as a Nurse Practitioner or Physician’s Assistant
- Basic Life Support (BLS) certification
- Advanced Cardiac Life Support (ACLS) certification
