Practice Nurse Tonbridge Primary Care Network
We are seeking a motivated Band 5 or Band 6 Registered Nurse to join our expanding Single Neighbourhood Team within the PCN. This is an exciting opportunity to work alongside a multidisciplinary team that includes Paramedics, Clinical Pharmacists, ANPs, and Care Coordinators, delivering high-quality, patient‑centred care.

Main duties of the jobThe successful candidate will play a key role in supporting our frailty cohort, including palliative care patients, housebound individuals, and those living with frailty. You will contribute to the delivery of Comprehensive Geriatric Assessments (CGAs) and work with GPs and the multidisciplinary team to coordinate ongoing care and treatment.
The role also involves delivering a range of nurse‑led clinics and housebound and care home visits, including vaccinations and, where appropriately trained, procedures such as FeNO testing, ECGs, and ear irrigation etc.
We welcome applications from experienced Band 5 nurses developing within primary care, newly qualified nurses with a strong interest in community and frailty care, and Band 6 nurses seeking a varied and autonomous role. You will be confident working independently across the PCN, demonstrate excellent communication skills, and support multidisciplinary working to improve patient outcomes.
This position offers an excellent opportunity to develop clinical skills, build strong relationships across practices, and make a meaningful contribution to integrated neighbourhood healthcare delivery.

Tonbridge PCN strives to develop collaborative working across Tonbridge GP practices and local health and care services. Tonbridge recently won a Kent and Medway award for its Integrated Home Visiting Service, jointly staffed by PCN GPs, paramedics and local community nursing teams.
The PCN aims to support and strengthen delivery of primary care, offering high standards of patient care centred on the needs of our population, whilst working to support a happy and resilient workforce through the development of robust multi‑disciplinary teams. A Same Day Access PCN Hub service with GP supervision using ACP clinical skills is currently in development, and we hope this will not only support patient access but provide a greater range of clinical experience for ACPs working at PCN level.
About West Kent Primary Care
West Kent Primary Care is an innovative GP Federation whose shareholders include all 50 practices in West Kent, covering nearly 500,000 patients. Within this, the practices are divided into 9 Primary Care Networks (PCN). We are made up of a core operations team, a PCN based workforce made up of clinical and non‑clinical staff and a Board of Executive Directors consisting of four local GPs and twelve PCN clinical directors.
The federation also offers a variety of services including, but not limited to, the Community Diabetes Service and a Frailty Service.
- Support public health and screening programmes, including immunisations, vaccinations, where appropriate.
- Completing CGAs and Respect forms.
- Deliver expert nursing assessments and manage complex care needs of housebound and care home residents within Tonbridge PCN in partnership with GP.
- Support patient and public involvement to enhance patient care and service delivery.
- Deliver a range of nurse‑led clinics and undertake housebound and care home visits, including providing vaccinations and, where appropriately trained, carrying out procedures such as FeNO testing, ECGs, ear irrigation, and other clinical interventions.
- Create a trusting partnership with patients and/or relatives/carers to communicate and explain complex medical issues
- Mentor, support, and supervise new‑to‑general‑practice nurses, student nurses, and other healthcare professionals (e.g., nursing associates, HCSWs) across the PCN by agreement and when appropriate.
