The successful candidate will play a key role in supporting our frailty cohort, including palliative care patients, housebound individuals, and those living with frailty. You will contribute to the delivery of Comprehensive Geriatric Assessments (CGAs) and work with GPs and the multidisciplinary team to coordinate ongoing care and treatment.
Responsibilities- Deliver Comprehensive Geriatric Assessments (CGAs) and complete Respect forms.
- Coordinate ongoing care and treatment with GPs and multidisciplinary teams.
- Deliver nurse‑led clinics and conduct housebound and care home visits.
- Provide vaccinations and carry out procedures such as FeNO testing, ECGs, ear irrigation and other clinically appropriate interventions.
- Support public health and screening programmes, including immunisations.
- Advocate for people and nursing and promote patient and public involvement.
- Mentor, support and supervise new‑to‑general‑practice nurses, student nurses, nursing associates and health care support workers within the PCN.
- Use digital technologies to enhance personalised care.
- Undertake mandatory training and CPD to meet NMC revalidation and practice needs.
- Provide clinical support and guidance to care home staff to improve patient safety and health outcomes.
- Ensure adherence to CQC standards, infection prevention and control protocols, and safeguarding responsibilities.
- Participate in audits, significant event reviews and quality improvement initiatives.
- Maintain confidentiality, information governance and data protection in line with NHS standards.
- Reflect on learning using appropriate frameworks (e.g., NMC Code) and keep practice updated.
- Registered Nurse with a Degree in Nursing or equivalent.
- Registration with the Nursing and Midwifery Council (NMC).
- More than one post‑registration qualification and ongoing CPD (e.g., certificates, diplomas at level6 such as long‑term conditions care, public health initiatives).
- Clinical examination skills.
- Proficiency in using clinical systems (e.g., Emis) and Microsoft
365 products (Outlook, Word, Excel). - Clean driving licence and access to a vehicle.
- Willingness to travel to care homes within the Ridge locality.
- Ability to use digital technologies applicable in the workplace to enhance personalised care.
- Management of patients with long‑term and complex needs.
- Understanding of a range of assessment methods.
- Knowledge of public health issues in the area.
- Project management and clinical leadership experience.
- Primary Care or Community nursing experience.
- Nurse prescribing experience (where applicable).
- Audit, research and quality improvement skills.
- Conflict management, communication, compassion and integrity.
- Ability to combine theory and practical skills to develop nursing practice and judgment.
- Disclosure and Barring Service Check required; the post is subject to the Rehabilitation of Offenders Act (Exceptions Order) 1975.
- Regulated by the Nursing and Midwifery Council – must adhere to the NMC Code and continue professional development.
