Position Summary Rocky Mountain Care is seeking a compassionate and skilled Registered Nurse (RN) to join our Home Health team. The Home Health RN provides high-quality nursing care to patients in their homes, helping them maintain independence, manage chronic conditions, recover from illness or surgery, and improve overall health outcomes. The ideal candidate is patient-focused, organized, and committed to delivering exceptional care while collaborating with physicians, caregivers, and interdisciplinary healthcare teams.

Key Responsibilities Perform comprehensive patient assessments and develop individualized care plans.

Provide skilled nursing care in the home setting, including wound care, medication management, disease management, and patient education.

Monitor patient progress and update care plans as needed.

Coordinate care with physicians, therapists, social workers, and other healthcare professionals.

Educate patients and family members on treatment plans, medications, disease processes, and self-care techniques.

Complete timely and accurate documentation in the electronic medical record (EMR).Assess home environments to identify safety concerns and recommend appropriate interventions.

Respond to changes in patient conditions and communicate effectively with the care team.

Ensure compliance with federal, state, and organizational regulations and standards.

Participate in case conferences and team meetings to support positive patient outcomes.

Required Qualifications Current and unrestricted Registered Nurse (RN) license in the State of Utah.

Graduate of an accredited nursing program.

Minimum of one (1) year of nursing experience required.

Current BLS (Basic Life Support) certification.

Valid driver's license, reliable transportation, and proof of auto insurance.

Ability to travel throughout assigned service areas.

Strong clinical assessment and critical thinking skills.

Excellent communication and interpersonal abilities.

Ability to work independently while managing multiple patient visits and priorities.

Preferred Qualifications Previous Home Health, Hospice, Skilled Nursing, or Case Management experience.

OASIS documentation experience.

Experience with Medicare and Medicaid home health regulations.

IV therapy and wound care experience.

Physical Requirements Ability to lift, transfer, or assist patients as necessary.

Frequent standing, walking, bending, and driving between patient homes.

Ability to safely navigate various home environments.

Ability to utilize standard nursing and medical equipment.
