Midland Care Connection is a not‑for‑profit, community‑based organization in Topeka, Kansas, dedicated to helping older adults and seriously ill individuals stay independent and supported in their homes. Established in 1978, we offer a full continuum of care, including hospice, palliative care, home health, grief and loss counseling, and our signature PACE (Program of All-Inclusive Care for the Elderly) program. Our mission centers on “meeting individual needs through innovative care solutions,” and we serve people across multiple counties in northeast Kansas.

The PACE program is designed to help older adults remain safely in their homes rather than entering nursing facilities. It provides complete medical care, rehabilitation, medications, transportation, social services, and supportive therapies—all coordinated by an interdisciplinary care team. Participants receive personalized care plans tailored to their unique needs, along with access to a PACE center for social activities, meals, and clinical services.

By integrating medical and supportive care under one program, PACE helps seniors maintain independence, improve quality of life, and continue living in their communities with dignity.

An Occupational Therapist in our PACE program helps participants maintain independence and safely manage daily activities. They assess each participant’s physical, cognitive, and functional abilities, then develop personalized plans to improve skills such as dressing, bathing, cooking, and mobility. OTs recommend adaptive equipment, home modifications, and safety strategies to prevent falls and injuries, while also addressing cognitive and fine‑motor challenges. They work closely with the interdisciplinary care team to support participants in remaining safely at home and engaged in meaningful daily activities.

Hours
:
Monday‑Friday, 8:00 am‑5:00 pm

• Master’s or Doctorate degree in Occupational Therapy from an accredited program
• Current state licensure as an Occupational Therapist
• Certification by the National Board for Certification in Occupational Therapy (NBCOT)
• Knowledge of geriatric care and rehabilitation principles
• Ability to assess functional, cognitive, and physical abilities
• Strong documentation and communication skills
• Ability to work collaboratively within an interdisciplinary healthcare team

• Experience working with older adults or frail populations
• Previous experience in a PACE, home health, rehabilitation, or long‑term care setting
• Knowledge of adaptive equipment, home modifications, and fall‑prevention strategies
• Experience with cognitive impairment or dementia care
• Bilingual or multicultural experience (depending on community needs)
