Integrated Health Program Manager

• Build and maintain collaborative relationships with community health partners, providers, and state agencies to drive integrated care strategies
• Lead planning, development, and implementation of programs that enhance service coordination between primary and behavioral healthcare systems
• Identify training needs, develop knowledge resources, and evaluate effectiveness of integration-related programming across the state
• Collect and report data, synthesize findings from quality reviews, and provide insights for continuous improvement and sustainability planning

• Minimum 4 years of experience in healthcare, public health, social services, or a related field, with a background in project oversight or program implementation
• Strong interpersonal and communication skills; able to coordinate across teams and communicate with leadership and provider networks
• Proficient in Microsoft Office tools including Excel, Word, Outlook, SharePoint, and Teams
• Proven ability to think critically, solve problems, manage deadlines, and deliver results in a dynamic, stakeholder-driven environment

Hourly: $45 - 50/hr
