This role would commute to both Merriam and Topeka.
At Iconic Care, we are dedicated to improving the quality of life and therapeutic outcomes for patients in skilled nursing and long‑term care facilities. By collaborating closely with the primary care teams in these facilities, we increase patients’ ability to participate in physical therapy while reducing the need for pain medications.
We’re HiringWe are seeking a passionate, motivated provider to join us full‑time and independently offer care and consultations at our contracted facilities.
Day-to-Day ResponsibilitiesAs a physiatrist, you will round at 1–3 Skilled Nursing Facilities or similar rehabilitation centers, 1–3 days per week at each location. Your primary focus will be delivering specialized physiatry care to 25+ patients per day who are experiencing difficulty participating in therapy due to pain or functional limitations.
Key Responsibilities- Perform focused assessments and develop individualized treatment plans in collaboration with facility primary care teams to optimize therapy participation and functional recovery.
- Provide targeted interventions such as joint injections, Botox treatments, dry needling, and medication management based on patient‑specific needs.
- Serve as the sole physiatrist on‑site, operating with a high level of autonomy while supported by a collaborative clinical team for consultation as needed.
- Coordinate care and refer to other specialties or services for issues beyond the scope of physiatry.
- Current, unrestricted PA or NP license.
- DEA licensed or willing to obtain DEA licensure.
- Nursing home and/or physiatry experience is a plus, but not required.
- Comfort and interest in performing peripheral joint injections (knees, shoulders, hips).
- Strong communication skills, with the ability to collaborate with multidisciplinary teams.
- Familiarity with EHR charting.
- A genuine passion for providing professional, personalized care in an autonomous environment.
- Hands‑on training in facility settings to ensure consistent, high‑quality care.
- Flexible 5 day workweek, with flexible hours. Weekends optional, no nights or on‑call shifts.

- Set patient load, with focused consulting visits for 23–25 patients/day after ramp‑up.
- Full autonomy in patient care with direct impact on patient outcomes at multiple facilities.
