RN Case Manager

About the Role

We’re looking for an RN Case Manager in Topeka, KS. The RN Case Manager will support the team with on‑call coverage, case management, and visits, helping to cover call‑outs.

$3,000 SIGN ON BONUS!!!

• Completing an initial assessment of the patient and family to determine hospice care needs.
• Regularly re‑evaluating the patient’s nursing needs.
• Using assessment data to determine care plan needs.
• Collaborating with the entire healthcare team, patient, and family to help direct the work of the team to provide continuous, well‑coordinated care.
• Counseling the patient and family in meeting end‑of‑life needs.
• Initiating appropriate preventative and palliative nursing procedures.
• Communicating with the physician regarding the patient’s needs and reporting any changes in the patient’s condition.
• Supervising the Hospice Aides.

• Associate’s Degree in Nursing.
• State‑certified RN license.
• Two years’ experience as a professional nurse; hospice experience preferred.
• Valid driver’s license and reliable transportation.
• Willingness to travel.
• Strong verbal and written communication skills.
