You’ll connect with patients through telephone and virtual visits, in the clinic, and occasionally in their homes, helping assess and address care needs, support medication management, provide education, and coordinate services.

While the position focuses largely on care coordination and clinical support, it also includes some direct patient care as needed. You’ll collaborate with nurses, providers, pharmacies, community agencies, and other healthcare professionals to ensure patients’ needs, goals, and care plans are effectively addressed.
