Midland Care Connection Midland Care Connection is a not-for-profit, community-based organization dedicated to helping older adults and seriously ill individuals stay independent and supported in their homes. Established in 1978, we offer a full continuum of care, including hospice, palliative care, home health, grief and loss counseling, and our signature PACE (Program of All-Inclusive Care for the Elderly) program. Our mission centers on  meeting individual needs through innovative care solutions,  and we serve people across multiple counties in northeast Kansas.

Palliative Care Program Midland Care Connection’s Palliative Care program is dedicated to providing compassionate, holistic care to individuals living with serious or life-limiting illnesses. Our team focuses on comfort, dignity, symptom management, and quality of life while supporting patients and their families through every step of the journey. Through an interdisciplinary approach, Midland Care provides personalized medical, emotional, spiritual, and practical support in the setting that best meets each patient’s needs.

Palliative Care LPNAs a Palliative Care LPN, you'll provide compassionate nursing care to seriously ill patients and their families while working closely with Midland Care’s interdisciplinary Palliative Care team. This role is primarily office-based and includes setting up and maintaining patient charts, coordinating care activities, managing telephone communications, and conducting patient follow-up calls to monitor needs and support continuity of care.

You'll connect with patients through telephone and virtual visits, in the clinic, and occasionally in their homes, helping assess and address care needs, support medication management, provide education, and coordinate services. While the position focuses largely on care coordination and clinical support, it also includes some direct patient care as needed. You'll collaborate with nurses, providers, pharmacies, community agencies, and other healthcare professionals to ensure patients' needs, goals, and care plans are effectively addressed.

This role also involves maintaining accurate documentation, participating in care planning and quality improvement activities, and building meaningful, therapeutic relationships with patients and families throughout their care journey.

Required Current, unrestricted Kansas LPN license

Valid Kansas Driver’s License Valid automobile liability insurance and a driving record that meets Midland Care’s insurability standards

At least one year of experience as an LPNAt least one year of experience working with frail or elderly populations

Ability to relate well with seriously ill individuals and their families

Basic computer literacy and ability to use electronic medical records and other required technology

Preferred

Experience providing care in a home health or community health setting

Experience working as part of an interdisciplinary healthcare team Experience or knowledge related to palliative care nursing

Strong communication and interpersonal skills, with the ability to establish therapeutic relationships with patients and families from diverse backgrounds

Flexibility to respond to changing patient needs, schedules, and assignments
