Service Coordinator (Weeknights + Every Weekend Required)
We are seeking a driven, organized professional to help people with complex health needs live happy, independent lives in their homes and communities.
As a Service Coordinator, you will coordinate and schedule client service plans, communicate with clients, intake specialists, and customer service teams, and ensure timely, seamless care.
Positions: Three full‑time, permanent Service Coordinators
Salary: Commensurate with experience
Hours: 34 hours/week. Hybrid work‑from‑home role with the following shift options:
Monday 12:30‑8:30 pm;
Thursday 12:30‑8:30 pm;
Friday 1:30‑8:30 pm;
Saturday & Sunday 7:00‑3:00 pm;
Wednesday & Thursday 12:30‑8:30 pm;
Friday 1:30‑8:30 pm;
Sunday‑Wednesday 12:30‑8:30 pm with Friday 1:30‑8:30 pm.
Reports to: Supervisor, Client Services
Responsibilities- Coordinate and schedule client service plans within a specific geographic area, reviewing plans to establish priorities.
- Optimize efficiency by continually reviewing PSW schedules.
- Collaborate with Intake Specialists and Client Service Supervisors to manage client expectations.
- Develop, manage, and update service delivery schedules based on client needs.
- Document and provide necessary information to the service delivery team.
- Build trust‑based relationships with staff, clients, and family members to meet their needs and resolve concerns.
- Provide accurate, courteous client service via telephone, written, and email communication.
- Troubleshoot problems, handle difficult situations, and escalate to Intake Specialist, CSS, or management when needed.
- Complete all relevant paperwork accurately and timely.
- Maintain up‑to‑date records in the electronic data system in accordance with policies and legislation.
- Notify stakeholders (Service Delivery staff, funders—LHIN) as required.
- Respond promptly to voicemail, email, and calendar communications.
- Participate actively in the Client Services Team, collaborating with peers and seniors to achieve strategic goals.
- Work with external partners (LHIN, Baycrest, government agencies) to support service delivery.
- Lead by example and demonstrate organizational values in every interaction.
- Post‑secondary education in a health‑related field preferred.
- Minimum one year of experience in community health care service coordination.
- Additional language skills considered an asset.
- Extensive knowledge of community resources and supports.
- Excellent telephone, interpersonal, and customer service skills.
- Strong organizational and time‑management skills.
- Ability to work efficiently in a high‑pace environment and manage stress.
- Good judgment in assessing difficult situations.
- Capacity to perform routine work independently.
- Knowledge of general office practices, procedures, standards, and medical terminology preferred.
- Proficiency in Microsoft Office applications preferred.
