Position Summary

This position is expected to work closely with SRCHC partner organizations and is responsible for supporting program implementation, strategic leadership and overseeing clinical care for clients accessing services. Physicians will contribute to clients’ health and well-being through assessment, prevention, education, rehabilitation and or/ therapeutic support services consistent with the standards of care outlined by the college of Physicians and Surgeons of Ontario.

These services are part of an interdisciplinary team approach to client care. Services are provided to clients, target community groups, and priority populations identified by the SRCHC Governance Committee.

At South Riverdale Community Health Centre, the team recognizes that health is shaped by a broad range of social and economic factors, including education, employment, food security, and social support. SRCHC takes a holistic, community health approach, to promote health/well-being, prevent illness, and strengthen community capacity and connection. Responsibilities

Direct Client Care
  • Ensures that standards of client care are maintained according to accepted College of Physicians and Surgeons of Ontario standards and policies of SRCHC and the Regulated Health Professions Act
  • Apply anti-oppression, harm reduction, and equity principles in all clinical care, advocating for client rights and health equity
  • Provide clinical leadership in the care of clients with complex medical, psychosocial, mental health, and substance use needs, including coordination with internal teams and external providers
  • Provides evenings, occasional weekends and after-hours medical care for clients in rotation with other MDs
  • Makes internal referrals to other SRCHC staff and community partners as appropriate
  • Refers to therapeutic counselling or short-term crisis management when required
  • Performs virtual consultation, when necessary, which may include assessment and advice
  • Incorporates health care advances into practice and bases this practice on the changing demographics and evolving needs of the community
Health Promotion and Education
  • Collaborates with SRCHC service team and other teams across the organization to identify and provide health education to target groups
  • Participates in community projects or agency activities as a representative of SRCHC and the Health Centre as needed
  • Assists in the development of community outreach programs based on the needs of individuals and the community
  • Participates in the development of, and gives support to policies and strategies that are in the interest of public health and recognize the determinants of health.
Organizational Responsibilities
  • Maintains complete and accurate records sufficient for timely communication with other professionals
  • Ensures timely completion of statistical and electronic health information as required
  • Participates in all relevant SRCHC and SRCHC programs, activities, meetings, professional associations, and workshops as required
  • Participates in the development of clinical policies and procedures as required
  • Contributes to the team’s use of research methodologies and findings in clinical practice, and to planning, monitoring and evaluation
  • Performs clinical management tasks in alignment with Infection Prevention and Control (IPAC) procedures
  • Maintains current and appropriate college membership as regulated under the Regulated Health Professions Act, 1991
  • Maintains malpractice insurance with the Canadian Medical Protective Association
  • Maintains continuing medical education (CME) credentials as expected by CPSO and all other required memberships in order to practice
  • Supports and/or supervises student placements within SRCHC and SRCHC as required
  • All other duties as required
Qualifications
  • Doctor of Medicine degree from a recognized university
  • Current registration and in good standing with the College of Physicians and Surgeons of Ontario (CPSO)
  • Membership and Certification from the College of Family Physicians of Canada (CFPC)
  • Member of Ontario Medical Association (OMA)
  • Member of the Canadian Medical Protective Association (CMPA)
