SE Health, in collaboration with Journey Home Hospice, is seeking a physician to join our Toronto program serving people experiencing homelessness with hospice palliative care. The role is fee-for-service with two-week rotations, including on-site visits and on-call hours, within a trauma-informed, culturally safe team.

You will work with an interdisciplinary team to provide patient-centered care, participate in ongoing research, and contribute to a growing program that prioritizes accessibility
