Job Description

The Mount Sinai Academic Family Health Team – Toronto site is looking for a Nurse Practitioner to join the team to support us on our journey. In collaboration with the interdisciplinary team, the Nurse Practitioner – Primary Care will be responsible for providing primary care to patients in the Mount Sinai Academic Family Health Team. The Nurse Practitioner will perform clinical duties within their scope of practice, including assessments, diagnosis, treatment, follow up, counseling, screening, education and referral.

These duties will be performed at the clinic through scheduled virtual and in‑person appointments for all patients at this site of our Academic Family Health Team.

This role will also be a key member of the Mount Sinai Academic Health Team’s Healthy Living with Chronic Pain (HeLP) program. This program aims to address the challenge of providing high‑quality patient care for those living with chronic non‑cancer pain on opioid medication(s) in resident practices. The NP in this program will specialize in complex pain management using a holistic approach to assessing pain for each individual and taking into account not only specific disease processes but also the biological, psychological, spiritual, and sociological factors for each patient.

Job Requirements Mandatory
  • Completion of a Master’s degree in Nursing or equivalent NP program
  • Nurse Practitioner (Primary Health Care) Certificate or equivalent
  • Current registration (Extended Class) and in good standing with the College of Nurses of Ontario (CNO)
Preferred
  • Experience or eligibility for academic appointment
  • Membership in RNAO and other relevant professional organizations
  • Previous experience as an advanced practice nurse, educator, or in leadership roles
Skills and Knowledge
  • Proven expertise in the Nurse Practitioner role within primary care, including assessment, diagnosis, and management across the lifespan
  • Knowledge and experience in preventive care, chronic disease management, and population health principles
  • Strong clinical judgment, decision‑making, problem‑solving, and critical thinking skills
  • Ability to work independently while collaborating effectively within an interprofessional team
  • Excellent interpersonal, communication, and patient engagement skills, including working with patients and families in vulnerable situations
  • Demonstrated ability to support knowledge transfer, mentorship, and education of students and other health professionals
  • Ability to assist in the development, implementation, and review of clinical processes, procedures, and directives to ensure high‑quality, efficient patient care
  • Demonstrated commitment to evidence‑informed practice, continuous professional growth, and ongoing learning
  • Strong organization and time management skills
  • Proficient in electronic medical records and other digital tools supporting patient care
  • Knowledge of primary medication and non-pharmacological management of chronic pain
