Pharmacist 2 - Renal Unit Pharmacy

To provide a full range of pharmaceutical care for patients living with Chronic Kidney Disease throughout the continuum of care (inpatients/outpatients) at an advanced level.

• Medical history and medication reconciliation
• Physician order review, clarification, and entry
• Participation in multidisciplinary dialysis rounds and kidney care clinics
• Answer drug‑related questions and provide patient counseling
• Supervise and support renal pharmacy technicians
• Lead the development, implementation, and maintenance of patient education materials, medication policies, procedures and protocols

The successful candidate will participate in the advancement of patient care through inter‑professional rounds, unit‑based and organizational committees, quality improvement initiatives and practice‑based research.

The pharmacist will be required to provide occasional cross coverage and will work at both the Sunnybrook and CNIB sites to provide patient care.

• Current licensure by the Ontario College of Pharmacists
• Excellent interpersonal and communication skills

• Committed to excellence in all aspects of the discipline, including professional practice, research and education
• Knowledge, skills and experience in adult education, program design, teaching, group facilitation, teamwork and conflict management
• Previous hospital pharmacy experience and/or pharmacy residency preferred
• Previous experience working with nephrology patients would be an asset
