Join as a Speech Language Pathologist at SE Health
. The Acute Care Transitions Program ensures seamless, compassionate care as patients move from hospital to home. Designed to support both patients and their families, the program connects individuals with community-based healthcare providers, social support services, primary care, and acute care teams. Together we create personalized plans that improve outcomes and ease the transition to home.
We are hiring dynamic, passionate Speech Language Pathologists (SLPs) who want to be part of transformational change in home and community care by providing care in a truly client‑centered approach. You will be excited about working in an environment that puts the care team in the driving seat, being part of a self‑directed team that offers a high level of professional autonomy and responsibility.
You will serve a roster of clients in the community, conduct joint care planning focused on client‑centered goals, and collaborate with an interdisciplinary team to reach those goals.

- Conduct assessments.
- Formulate goals and recommendations.
- Plan and implement client‑centered, goal‑directed treatment programs.
- Graduate of an accredited Speech Language Pathology program and in good standing with the College of Speech Language Pathologists of Ontario.
- Recent experience managing patients in acute/rehab or community‑based rehabilitation programs is preferred.
- Reliable, flexible, self‑directed professional able to work effectively with rehab assistants and an interdisciplinary team.
- Sound knowledge of community resources is an asset.
- Effective interpersonal, organizational and communication skills.
- Positive work and attendance record is essential.
- Ability to adapt to new computer applications.
- Hybrid work approach: remote and in‑service with clients in their settings.
- Valid driver’s license, vehicle and ability to travel.

Compensation and Additional InformationHiring pay range: $41.00 – $48.00 Paid Per Visit, based on experience. This role is a replacement position. All applications and interviews are reviewed by our Talent Acquisition team.
