* In this valuable role, the Clinical Supervisor is accountable for the supervision of PSW’s and HSW’s who are caring and making a difference for clients within the community. * What you’ll do:

*** Lead and support PSW’s and HSW’s to ensure practices comply with all legislated standards of practice and company internal protocols
* Respond to and mentor staff, patients/families, and other team members with regular touchpoints about concerns from community
* Provide ongoing leadership through regular huddles, face-to-face meetings and one-on-ones.
* Initiate patient safety incident investigations and documents to ensure timelines and action plans are met.
* Work collaboratively with nursing team members to support clinical documentation and submission requirements according to specific funder reporting guidelines.
* Engage patients in collaborative problem-solving by building trust, providing health education, and identifying their care needs, strengths, and goals
* Engage staff in problem solving, offering education, coaching, and mentoring in resolution of problems.
* Maintain the knowledge, skills, and attitudes to provide safe, efficient, effective service in all areas of practice by keeping abreast of changes in practice setting that affect scope of practice.

* What you bring:

*** Current RPN registration with the College of Nurses of Ontario and entitled to practice without restrictions
* Minimum of 5 years’ experience as a practicing Registered Nurse
* Comprehensive patient health assessment skills are required
* Possess an understanding of the principles of community nursing practice
* Proven experience leading and managing a team
* Knowledge of clinical documentation standards and practice guidelines is required
* Excellent organizational, interpersonal, and teaching skills
* Commitment to professional development in clinical practice that is relevant to home health care
* Effective critical thinking and problem-solving skills
* Must possess a valid driver’s license and access to reliable transportation
* Able to provide a current, clear Vulnerable Sector Screening (VSS)
* C.N.A. Certification as a Community Health Nurse (CCHN(C)) is considered an asset
