Rideau Community Health Services (RCHS) is a non-profit, community-governed health and social service agency that provides comprehensive primary care, health promotion and community development services. Working with partners to provide integrated health and wellness services for various communities throughout Leeds, Grenville and Lanark counties.
As a member of an inter-disciplinary team, the Registered Diabetes Nurse Educator provides community-based nursing functions including prevention and management of adult diabetes and participates in the development, implementation, monitoring and evaluation of programs and services for individuals, families and the communities that are affected by diabetes. The Registered Diabetes Nurse Educator will act as an ambassador for the Rideau Community Health Services Diabetes Education Program in the community and promote positive partnerships with other providers to maximize the capacity of Lanark, Leeds and Grenville to support the prevention and management of diabetes.
KEYFUNCTIONS
- Perform nursing assessments, development of client-focused diabetes management plans, health education/counselling and other clinical activities according to the College of Nurses of Ontario standards of practice and the current Diabetes Canada Clinical Practice Guidelines for the Prevention and Management of Diabetes.
- Work closely as a team with the client, RCHS DEP dietitians, primary care providers, outside social services agencies and administrative staff to provide a holistic, accessible, client-centred diabetes care plan for all clients.
- Make recommendations, in consultation with a Registered Dietitian (CDE), to primary care providers on medication and insulin changes for clients when the appropriate knowledge, skill and judgement has been demonstrated (eg, completion of CDE certification)
- Counsel clients on insulin initiation, adjustments and pattern management when the appropriate knowledge, skill and judgement has been demonstrated (eg, completion of CDE certification)
- Perform community needs assessments with the Dietitian and identify barriers to the prevention and management of diabetes and access to service.
- Plan and deliver diabetes in-services to local health care providers.
- Mentor/supervise nursing students as requested.
- Contribute to chart documentation, participate in chart reviews and case conferences, and make referrals.
- Work in partnership with other agencies to support initiatives related to diabetes prevention and management.
Provide support at events sponsored by RCHS DEP.

- Work during both regular and extended hours of operation in locations identified by RCHS DEP.
- Contribute to the RCHS practices of hiring, orienting, training and mentoring staff as requested.
- Support Occupational Health and Safety policies and procedures.
- Perform other related duties that are commensurate with the intent of this position.
Undergraduate degree or equivalent nursing experience (Registered Nurse)
- Registration with the College of Nurses of Ontario.
- Two to five years counselling or patient education experience preferred.
- Certified Diabetes Educators (CDE) certification or commitment to acquire.
- Experience in program development, implementation, monitoring and evaluation.
- Proficiency in the use of computers and various electronic health record applications.
- Use of a vehicle and a valid driver’s licence.
- Willing to obtain valid criminal reference check.
