Social Worker – Hospice, Grief & Bereavement 12 Month Contract About the Agency

Aboutthe Role
We are seeking a Social Worker – Hospice, Grief & Bereavement to provide direct counselling, case management, service coordination, case planning, and goal setting to clients using evidenced informed social work practice. This position will focus on the provision of service to clients and families facing life limiting illness and support healthy coping in grief and bereavement.
Key Responsibilities- Provide comprehensive biopsychosocial assessment, develop collaborative goals and create effective care plans with clients with life limiting illness and their caregivers/families.
- Complete risk assessments and crisis interventions within a psychosocial framework.
- Support Advanced Care Planning conversations (ACP) and provide related education to clients, caregivers and families
- Assess grief support needs from an evidence based model and tailor interventions (social, educational, clinical) appropriately
- Provide individual grief counselling and facilitate bereavement groups
- Maintain an understanding of relevant legislation such as Health Care Consent Act, Medical Assistance in Dying, PHIPA, Substitute Decisions Act and their application to practice.
- Understand and apply current norms of practice and best practices in Hospice Palliative Care and Grief and Bereavement.
- Perform other duties as assigned.
- Masters of Social Work
- Minimum 3 years related clinical experience
- Fluency in Russian and/or Hebrew are considered assets.
- Member of OCSWSSW in good standing
- Valid Ontario Driver’s License and access to a vehicle
- Experience working within the health care system, preferably in a Hospice Palliative Care setting
- Excellent communication, organization and interpersonal skills
- Knowledge of Jewish culture, community needs and resources considered an asset
