Role:
Clinical Pharmacist
To provide direct patient care in the management of drug therapy to achieve positive patient health outcomes, primarily for patients of a medical-type inpatient unit, and coverage to other areas as needed. Direct patient care focuses on review of medication orders; selection and assessment of patients requiring pharmaceutical care; identifying, resolving and preventing drug therapy problems for a subset of patients;
patient education; patient continuity of care; and the provision of drug information.
To facilitate and promote accurate, safe, appropriate and cost-effective use and distribution of drugs throughout the hospital, including weekend, evening and on call rotations.
To support and participate in educational and research programs sponsored by the pharmacy discipline or hospital.
To participate in the development, revision, and implementation of standards, protocols, procedures, guidelines pertaining to drug therapy and pharmacy practice.

Position Summary- Provision of direct, patient-focused care in a multi-disciplinary team environment.
- Provision of drug information and development and/or review of medication protocols specific to clinical practice.
- Support of the medication distribution system.
- Preceptorship of pharmacy students for on-site clinical education.
- Projects and safety initiatives
- Provision of education, presentations and in-services for the department of pharmacy and clinical program.
- Graduation from recognized pharmacy degree program.
- Licensure with the Ontario College of Pharmacists.
- Previous hospital experience in direct patient care in a team environment an asset.
- Hospital Pharmacy residency an asset.
- Self‑motivated team player, reliable, cooperative, and patient‑focused.
- Excellent interpersonal and communication skills.
- Excellent attendance record.
