Position Summary Dietitian, Diabetes Education Centre (DEC)The Registered Dietitian is a key member of the North York General inter professional health care team, providing safe, effective, and ethical nutritional care for patients and families. You deliver care in accordance with professional and hospital standards, working both independently and collaboratively. You bring strong leadership, communication, critical thinking, and organizational skills while maintaining accurate clinical documentation. This position also supports the hospital’s academic mandate and helps ensure a healthy and safe environment for patients, staff, and families.
The Diabetes Education Centre (DEC) provides inter professional care for pediatric and adult patients with type 1, type 2, and gestational diabetes, and you will apply in depth diabetes expertise to support patients, families, and fellow health professionals. Working collaboratively within an interdisciplinary team, you will deliver culturally sensitive, patient and family centred care focused on assessment, education, prevention, and health maintenance.
You will build strong relationships across the organization, demonstrating excellent communication and the ability to manage diverse patient needs. This position supports the hospital’s academic mandate by advancing care through teaching and learning and/or transforming care through research and innovation. This position contributes to the shared responsibility of ensuring health and safety policies are followed to provide for a healthy environment for patients, staff, physicians, volunteers and families.
a practical level, you will
- Provide comprehensive, patient- and family‑centered assessments and care, addressing emotional, physical, cognitive, and social needs for individuals living with diabetes and their families/caregivers.
- Deliver diabetes self management education and nutrition counseling to individuals, groups, families, and caregivers across all ages.
- Provide diabetes education to other health care professionals, consult with other health care teams and implement evidence‑based practices.
- Develop and implement individualized nutritional care plans and therapeutic diet prescriptions in collaboration with interprofessional teams, following best practice guidelines.
- Advocate for the patient/family and community to ensure the patients’ rights are recognized and appropriate services are created/provided in collaboration with the inter professional team.
- Coordi Coordinate and facilitate discharge planning with patients, families, and the health care team, ensuring effective use of relevant community resources.
- Lead and participate in center specific and corporate diabetes care initiatives, promoting evidence based practices aligned with organizational strategic priorities.
- Promote best practice and evidence‑based care through the implementation of initiatives aligned with the organization’s strategic direction. Support in the development of program/unit goals, objectives, policies, procedures and quality assurance programs.
