How You Will Make An Impact

The Intensive Case Manager will demonstrate a wide array of strengths including a compassionate and caring nature, in conjunction with an unbiased, non-judgmental and person-centered approach while working with clients to develop a level of trust so that clients feel secure and supported. This level of trust will be established and maintained by involving clients as much as possible in discussions around their goals and recovery process.

The professional relationship is grounded in regular assessment of client-centered needs and is a collaborative effort between client and ICM.   • Carry out all duties with a work ethic demonstrating leadership, role modeling of behaviours, and managing responsibilities in accordance with the Mission and Values of Good Shepherd Non-Profit Homes.
  • Provide service in a dignified, culturally competent and respectful manner that reflects Good Shepherd Centers' charism and an integrated anti-racism/anti-oppression framework.
  • Maintain a caseload within the provincial guidelines for Intensive Case Management.
  • Engage in the intake process including liaison with Access Point, interviewing potential clients, utilizing appropriate assessment tools to determine goals and needs, preparing client summary reports, discharge/transfer reports, maintaining files, progress notes and completing all other necessary paperwork.
  • Provide resource information, community facilitation, and advocacy for clients as needed; complete referrals as required.
  • Utilize a psychosocial rehabilitation approach to support clients in achieving goals that enhance their enjoyment and quality of life.
  • With client consent, liaise with clients' formal and informal supports.
  • Assist clients in developing necessary life skills as needed.
  • Help clients maximize their competencies to become more self-sufficient.
  • Work from a strengths-based perspective within an empowerment framework.
  • Keep the Program Manager informed of developments and changes concerning clients as needed.
  • Observe all health and safety rules and comply with the Occupational Health and Safety Act and its regulations.
What You'll Bring To Our Team
  • University Degree in Social Work, Nursing, Occupational Therapy or related field, plus an Addictions Certificate from a recognized college, plus two years of work-related experience, particularly with persons experiencing problematic substance use and mental health challenges.
  • Extensive knowledge of the social service system and community resources.
  • Expertise providing counselling and case management to individuals with complex substance use and mental health histories.
  • Crisis intervention and risk assessment skills, with well-developed problem-solving and interpersonal abilities.
  • Demonstrated knowledge of crisis intervention de‑escalation strategies and ability to remain calm in crisis situations.
  • Ability to recognize and respond to the emotional, psychological and physical needs of individuals in crisis.
  • Fluency in a second language is an asset.
  • CPR certification or willingness to be certified is required.
  • Commitment to anti‑racist/anti‑oppressive practice and to the mission and values of Good Shepherd.
  • Understanding of workplace health and safety practices and employee responsibilities under current legislation.
