Role Overview
Sinai Health is looking for a Social Worker to support our Float/Alternative Level of Care (ALC) team. Reporting to the Senior Manager, Academic Practice, Health Disciplines, this role functions as a hospital wide resource in a float capacity.
Responsibilities- Assess and provide interventions to support patient transition
- Provide unit coverage and backfill according to the Social Work coverage model
- Support complex transitions/discharge planning for patients designated ALC and/or at risk for ALC
- Support the referral process to the SHtoHome program
- Work in collaboration with interprofessional care teams
- Support the continued growth and evolution of the SHtoHome program or other ALC initiatives
- Perform other duties consistent with the job classification as required
- Maintain demonstrated satisfactory job performance and attendance history
- Mandatory:
- Master's Degree in Social Work (MSW)
- Currently registered and in good standing with the Ontario College of Social Workers and Social Service Workers (OCSWSSW)
- Preferred:
- Previous Social Work experience in a hospital setting
- Previous experience working with patients with complex needs, mental health and addiction issues
- Experience in providing supportive counselling to patients and essential care partners about complex discharge planning
- Strong understanding of community resources and options such as financial, housing and care settings available to support transition
- Knowledge of relevant legislation around discharge planning
- Vacancy Type:
New/Replacement Vacancy - Salary:
Minimum $44.67/hr;
Maximum $57.90/hr - Note:
Evening and weekend shifts may be required pending patient care and operational needs
