Position: Stem Health Care Coordinator Role
Join Stem Health as a Care Coordinator and Client Advocate, where your organizational skills will lead members through their health journeys. This role is essential for creating meaningful care experiences for clients.

Stem Health is a new, membership-based clinic set to open in September 2026 at First Canadian Place. We seek a dedicated Care Coordinator with a clinical background who excels at member advocacy and has a passion for facilitating healthcare navigation. Your expertise will help ensure each member receives personalized support throughout their care.

Key Responsibilities:

• Manage referrals and communication with community providers

• Proactively solve issues in members’ care pathways

• Build and maintain trustful relationships with members

• Participate in case reviews to align on care goals

• Ensure compliance with member information privacy

Requirements:

• RN or equivalent experience in a health-related field

• At least 3 years of experience in care coordination

• Strong understanding of Ontario's healthcare system

• Proficiency in digital health records

• Additional languages are beneficial
