Registered Dietitian and Diabetes Specialist

Take on a vital role as a full-time Registered Dietitian specializing in diabetes education r work will enhance patient care in both primary health settings and diabetes management.
In this position, you will assess complex patients at both the Sunnybrook Diabetes Education Centre and the Family Health Team.

Your role will include developing comprehensive nutrition plans, providing education on technology for self-management, and supporting interdisciplinary care initiatives. Join a dynamic team committed to improving patient outcomes and fostering lifelong learning.

Key Responsibilities:

• Conduct thorough nutrition assessments of patients

• Offer customized medical nutrition therapy

• Facilitate individual and group education for diabetes management

• Maintain accurate EMR records for each patient

• Lead community outreach and educational initiatives
Requirements:

• Bachelor's degree in Human Nutrition

• Active registration with College of Dietitians of Ontario

• Certification as a Diabetes Educator preferred

• Minimum 2 years relevant dietitian experience

• Valid CPR/BLS certification required
