This position allows you to assess and address nutritional needs through comprehensive patient evaluations, integrating cultural and psychosocial factors. Key Responsibilities:
• Assess patients' food habits and nutritional needs
• Design and implement effective nutrition therapy plans
• Maintain detailed electronic records of patient interactions
• Facilitate community nutrition education sessions
• Participate in quality improvement and program development
Requirements:
• Accredited Bachelor's degree in Nutrition
• Current registration with the College of Dietitians of Ontario
• At least 2 years relevant dietary experience
• Strong organizational and problem-solving skills
• Flexibility to work independently and within teams
