Position: Community Health Worker Open Door Program

Access Alliance Multicultural Health and Community Services seeks a dedicated Community Health Worker for their Open Door Program. This position emphasizes community engagement, outreach-based health initiatives, and improving health outcomes for immigrant and refugee populations.

Your role will include facilitating workshops, developing care plans, and creating resources to support preventive health efforts.

Key Responsibilities:

• Plan and implement workshops on chronic disease prevention

• Support individuals in creating preventive care plans

• Develop health information materials for community use

• Build a comprehensive resource list of services

• Collaborate with the team on community outreach strategies

Requirements:

• Undergraduate degree in Health Sciences or related field

• Minimum 3 years in community outreach or health education

• Strong knowledge of health issues impacting newcomers

• Experience coordinating community programs and events

• Excellent communication and interpersonal skills
