This part time Dietitian position, at 0.5 FTE, aims to create a welcoming environment will evaluate nutritional needs, plan health education sessions, and collaborate with the interdisciplinary care team. Your expertise will assist clients, particularly those with pre-diabetes and Type 2 diabetes, in developing health improvement plans tailored to their needs.
Key Responsibilities:
• Evaluate individuals' nutritional status and provide support
• Develop educational materials for various audiences
• Plan and conduct health education sessions effectively
• Provide assessment for clients with dietary concerns
• Ensure documentation is accurate and timely in records
Requirements:
• Registration with the College of Dietitians of Ontario
• Undergraduate degree in dietetics or related field
• 2-5 years of clinical experience in community nutrition
• Understanding of social determinants affecting health
• Experience working with Indigenous communities and trauma-informed care
