This position is based within the Home Care branch of Continuing Care that offers a range of services tailored to meet individual needs, including acute care, rehabilitation, and support for chronic and palliative conditions. These services are determined through personalized assessments to help clients remain in their homes, providing respite for caregivers and promoting independence and a higher quality of life.

  • Recent experience in screening, intake, and coordinating services for clients with complex health conditions transferring to a long-term care/home care setting
  • Experience providing advanced nursing assessments within the scope of a Registered Nurse

Desired knowledge, skills and suitability

  • Strong knowledge and experience in resources encompassing a range of services/ programs to meet complex client needs
  • Excellent interpersonal skills, communication, organization and flexibility
  • Excellent ability to build and maintain effective relationships and partnerships through collaboration
  • CPR Level C with AED (completed within the past 30 months)
  • Transportation of Dangerous Goods (completed within past 30 months)
  • Current registration with the Yukon Registered Nurses Association

Job requirements

This job requires you to perform, and/or be exposed to, the following:

  • Heavy lifting/bending/reaching during client transfer
  • Exposure to pets, allergens, infectious diseases, second hand smoke and chemicals in cleaning supplies
  • Travel during adverse weather conditions
