Community Health Manager at CANES

Lead impactful community health initiatives as the Manager of Community Health and Wellness us on enhancing seniors' care through interdisciplinary team leadership and program development while ensuring high-quality, client-centered services.

This full-time, permanent role reports directly to the Director of Transitions and Integrated Care. You will oversee a diverse team, including Social Work, Nursing, and Recreational Therapy, ensuring seamless service delivery. Your ability to foster partnerships with external providers and operationalize referral pathways will be crucial in furthering CANES’ mission of promoting dignity and quality of life for seniors.

Key Responsibilities:

• Develop integrated care models addressing seniors’ varied needs

• Lead and supervise interdisciplinary healthcare teams

• Ensure compliance with performance targets and standards

• Analyze data for quality improvement initiatives

• Build relationships within the healthcare community

Requirements:

• Degree in health-related discipline, RN designation preferred

• 5 years' management experience in healthcare

• Strong data analytics and reporting skills

• In-depth knowledge of senior health needs

• Valid driver’s license and clear police check
