We are looking for a skilled Social Worker with a Master's degree and OCSWSSW membership. Your expertise in crisis intervention and psychosocial assessment will help guide patients and families during challenging healthcare experiences.

Key Responsibilities:

• Deliver psychosocial assessments and counselling to patients and families

• Support patients coping with illness and treatment challenges

• Facilitate safe discharge planning with community links

• Collaborate with healthcare professionals for coordinated care

• Advocate for patients within health systems and communities

Requirements:

• Master’s degree in social work from an accredited university

• OCSWSSW membership in good standing

• 3-5 years’ acute care experience in social work

• Knowledge in grief, oncology, or palliative care

• Excellent problem-solving and communication skills
