Health Promoter at South Riverdale

Join South Riverdale Community Health Centre as a Health Promoter for the Mobile Health Clinic and Women’s Clinic in East Toronto. This full-time role focuses on health education, community outreach, and connecting marginalized populations to primary care.

As a Health Promoter, you will coordinate health education and social programs while working collaboratively in a multidisciplinary environment. Your work will primarily support individuals facing barriers, including those who are homeless or dealing with mental health issues. The role requires flexibility to meet community needs, including early mornings, evenings, and weekends.

Key Responsibilities:

• Coordinate outreach and health promotion initiatives

• Provide health education for diverse populations

• Facilitate social prescribing and internal referrals

• Collaborate with community groups for program development

• Manage Mobile Health Clinic operations

Requirements:

• Post-graduate degree in health or related field

• 3-5 years in health promotion/education

• Class ‘G’ Ontario driver’s license

• Experience with diverse communities

• Strong facilitation and communication skills
