As a key member of the Integrated Care Program, you will facilitate post-acute recovery for patients.
Your role includes conducting assessments and implementing individualized interventions for speech and swallowing challenges. By collaborating with an interdisciplinary team, you’ll ensure comprehensive care tailored to each client’s needs.
Key Responsibilities:
• Assess speech and swallowing interventions at home
• Implement dysphagia management strategies for clients
• Collaborate with a multidisciplinary team for effective care
• Promote a holistic approach to patient support
• Utilize digital health tools for documentation and intervention
Requirements:
• Registered with College of Audiologists and Speech Language Pathologists of Ontario
• Valid driver's license and vehicle insurance
• Commitment to person-centered care principles
• Proficient in both in-person and virtual service delivery
• Strong communication and critical thinking skills
