VHA Home Health Care (VHA) in Toronto Central is seeking a Community Access Physiotherapist to join the UHN@Home Transitions Program. The role provides physiotherapy consultation, assessment, and short-term intervention for patients transitioning from hospital to home, within a structured 16-week care model aimed at stabilization and safe discharge.
