Elevate your career with Providence Care as a Community Social Worker. This full-time role focuses on delivering personalized support for adults with mental health challenges and neurocognitive disorders.
You will collaborate with an interprofessional team to create and implement individualized recovery plans. This position requires a commitment to advocacy, service coordination, and the development of client-centered assessments. Your work will facilitate safe transitions and enhance person-centered care for individuals needing support.
Key Responsibilities:
• Build effective therapeutic relationships with clients
• Lead the coordination of rehabilitation and support services
• Write and evaluate recovery goals in collaboration with clients
• Conduct comprehensive client-centered assessments
• Provide education on illness management and coping skills
Requirements:
• Master’s Degree in Social Work
• Active registration with the Ontario Social Work College
• One year experience in mental health care
• Skills in diverse community settings
• Valid Ontario G Class driver’s license
