As part of the EYRND Ontario Health Team, you will identify unattached individuals and strengthen ties across various health care providers. The role emphasizes engagement through community outreach and collaboration with both patients and providers to ensure seamless access to necessary health services. Your contribution will enhance overall care coordination in the community.

Key Responsibilities:

• Work closely with Health Care Connect for patient identification

• Act as a community resource for primary care navigation

• Organize community engagement and outreach initiatives

• Connect patients to appropriate primary care facilities

• Support project milestones and reporting as necessary

Requirements:

• Minimum 5 years of relevant experience

• Bachelor’s degree in a health-related area

• Background in community engagement

• In-depth understanding of Ontario’s health system

• Exceptional interpersonal and communication skills
