This role is critical in supporting patients navigating illness through psychosocial assessments and emotional support. As a Social Worker, you will engage with families and connect them to vital community resources. Your effective collaboration with an interprofessional healthcare team will enhance patient safety and discharge planning, fostering improved health outcomes.

Key Responsibilities:

• Perform thorough psychosocial assessments and emotional support

• Assist patients and families with coping strategies for illness

• Coordinate discharge planning connecting families to resources

• Work closely with healthcare team for patient-centered care

• Advocate for patients through the healthcare system

Requirements:

• Master’s degree in social work; OCSWSSW membership required

• 3-5 years of relevant experience in acute care settings

• Preferred experience in crisis intervention or oncology

• Strong skills in psychosocial assessment and communication

• Flexibility to work under pressure in fast-paced environments
