Care Partnerships Lead at Mino Care

Mino Care seeks a Care Partnerships Lead to enhance perinatal health through strategic partnerships in a hybrid setting. This role focuses on collaboration and health system navigation.

As the Care Partnerships Lead for Mino Care, you will oversee the development and sustenance of partnerships within the perinatal and reproductive health sector. Drawing on your experience in health system coordination, you will help establish referral agreements and ensure operational integration of Mino Care teams within partner institutions.

Key Responsibilities:

• Formalize referral partnerships across health and social services

• Negotiate and design partnership agreements and workflows

• Facilitate onboarding of Mino Care staff at partner sites

• Foster implementation of safe navigation pathways

• Track performance and reporting metrics for partnerships

Requirements:

• 3–5+ years in partnership management or health services

• Deep understanding of health systems and referral models

• Experience with hospitals and community health centers

• Commitment to culturally safe practices

• Excellent negotiation and leadership capabilities
