Key Responsibilities:
• Oversee coordinated care services for Community Members
• Conduct regular wellness assessments and develop care strategies
• Lead care conferences with families and healthcare providers
• Maintain service delivery documentation and health records
• Administer medications per regulatory standards
Requirements:
• Valid RPN license with a minimum of 3 years’ experience
• Proficient in clinical assessment and care planning
• Strong communication and interpersonal skills
• Experience with Microsoft Office Suite
• CPR/First Aid Certification required
