Key Responsibilities:

• Conduct assessments to determine patient eligibility for services

• Develop and coordinate tailored individual care plans

• Build supportive relationships with patients and families

Requirements:

• Fluency in both French and English

• 1+ years of relevant community health experience

• Knowledge of healthcare systems and community resources

• Strong communication and problem-solving abilities

• Valid driver’s license and access to transportation
