Position: Social Worker for ACT Team at SHN
Provide impactful support as a Social Worker (BSW) on the ACT Team at Scarborough Health Network. Deliver case management and treatment services focused on mental health and substance use.

This full-time role reports to the Manager of Adult Outpatient Mental Health and Substance Use Programs. Successful candidates will leverage their clinical skills over a minimum of three years in community mental health settings. You'll engage in assessments and management for clients with severe mental illnesses and collaborate within an integrated interdisciplinary team.

Key Responsibilities:

• Conduct comprehensive assessments for program clients

• Manage treatment planning and execution

• Support individuals facing severe mental health challenges

• Collaborate in a multidisciplinary team environment

• Provide crisis management and suicide risk assessment

Requirements:

• Bachelor of Social Work, registered in good standing

• Minimum three years of recent community mental health experience

• Proficiency in clinical documentation systems

• Strong interpersonal and communication skills

• Valid Ontario Driver's License and clean driving record
