Mental Health Social Worker at Humber River Health

This position supports clients through tailored psycho-social assessments and therapies, promoting their recovery journey. You will engage with patients and families, coordinate discharge plans, and ensure comprehensive patient documentation. Experience in mental health or addiction settings is essential for this role, as you will work collaboratively within a dynamic treatment team.

Key Responsibilities:

• Conduct thorough addiction assessments for referred patients

• Apply diverse psychotherapeutic approaches in therapy sessions

• Develop discharge plans with the treatment team

• Document patient progress according to standards

• Provide crisis intervention effectively during challenges

Requirements:

• MSW from an accredited institution

• Membership with OASW and OCSWSSW required

• Minimum two years of relevant experience preferred

• Knowledge of CBT, DBT, and Motivational Interviewing

• Strong interpersonal and decision-making skills required
