UHN is filling three permanent pharmacist positions at Toronto Western Hospital. As a vital member of the healthcare team, you will conduct medication histories, resolve therapy issues, and ensure effective drug monitoring. Patient education is crucial to your role, along with supervising pharmacy technicians.
Key Responsibilities:
• Conduct best possible medication history interviews
• Promote safe and effective medication use
• Monitor adherence to evidenced-based guidelines
• Educate patients regarding their medications
• Support clinical teaching within the pharmacy team
Requirements:
• Must have a Bachelor’s or Doctor of Pharmacy degree
• Active registration with the Ontario College of Pharmacists
• Prior hospital pharmacy experience is necessary
• Strong verbal and written communication skills
• Effective organizational and time management capabilities
